Each enquiry to VPIS is registered at the time of enquiry on an individual report form. Information collected includes:

These details allow some risk assessment to be made before information is provided.
Callers are asked to provide written follow-up information for the majority of enquiries received and they are sent (within 14 days of the original contact) a questionnaire, with pre-paid reply envelope. Returned questionnaires allow further details of clinical effects and treatments to be recorded along with the case outcome. They also allow verification of the data recorded at the time of enquiry and some assessment of the effectiveness of the VPIS response to the enquirer.
